The principal goal of this paper is to share program ideas and techniques with those involved in the development of transitional programs for psychogeriatric patients. While it emphasizes the experiences at Springfield Hospital Center in Sykesville, Maryland, its purpose is not to replicate the model but to encourage the refinement of programs that address psychogeriatric needs during transitional programming. This paper illustrates a four stage psychogeriatric transitional program which underscores the role of occupational therapy in providing continuity of care. Treatment progresses from the clinic setting, to a partial day program, to a full day program, to a visiting community rehabilitation program, to the discharge setting. It will discuss the role of occupational therapy in consultation and in overcoming common obstacles to discharge planning. |
